Discover App: Forms and Checklists

There are two different types of Forms that can be used to create various checklists for Teachers and Admins to use on the Discover App.

Centre Forms

Centre Forms can be used to create Excursion checklists, Opening/Closing/Safety logs, Sleep Room Supervision Charts or Custom Roll Calls.
These types of forms are best suited to processes/procedures, as they do not include Child names as a field.

Navigate to Centre Set Up > Setup Centre Forms

  1. Select 'Create New'.
  2. Type in a Form Name and Description.
  3. Click 'Add Field'.
  4. Type in the Field Name (this will be one of the items in your checklist).
  5. Add a Field Tool tip if you would like to add more information about the field.
  6. Select the Field Type from the dropdown.
  7. Add the Report Title to indicate how you would like the field to be displayed on reports.
  8. Tick 'Is Required' if necessary.
  9. Add in List Options if you have opted for a Listy in the Field Type.
  10. Click 'Create'.
  11. Repeat Steps 3-10 as needed to add in the items for your checklist.


To view recorded data from a Centre Form, navigate to Reports > Centre Reports > Centre Tablet Forms.


Child Forms


Child Forms can be used to create Sleep/Nappy/Toileting checklists, completed/signed form checklists etc.
These types of forms are best used as checklists that need to be completed on a child-by-child basis.

Navigate to Centre Set Up > Setup Child Forms

  1. Select 'Create New'.
  2. Type in a Form Name and Description.
  3. Click 'Add Field'.
  4. Type in the Field Name (this will be one of the items in your checklist).
  5. Add a Field Tool tip if you would like to add more information about the field.
  6. Select the Field Type from the dropdown.
  7. Add the Report Title to indicate how you would like the field to be displayed on reports.
  8. Tick 'Is Required' if necessary.
  9. Add in List Options if you have opted for a Listy in the Field Type.
  10. Click 'Create'.
  11. Repeat Steps 3-10 as needed to add in the items for your checklist.

To view recorded data from a Centre Form, navigate to Reports > Children Reports > Children Tablet Forms.


Using Forms

Once created, forms can be filled out using the Centre Tablet.


  1. When logged in, navigate to either 'Centre Forms' or 'Child Forms'.
  2. Select the Form from the dropdown.
  3. Select the Date and Room (if applicable).
  4. For Child forms, the checklist will be completed against each child signed in to the centre on the selected date.
  5. When the checklist has been completed, select 'Save' at the bottom of the screen.
