Children: Online Enrolment Form

Administrators will be able to locate, review and edit their enrolment forms within their Discover system. To locate:


1. Navigate to Centre Setup 

2. Select the Set Up Widget tab

3. From within this screen, select Enrol at this Centre 



Administrators can copy the link found here into any communications to send to families. Alternatively, this link can be housed on the centre website for families to locate and complete independently.

Customising the Online Enrolment Form

Various sections of the online enrolment form can be updated to meet any centre specific requirements.

To make change, navigate to Centre Setup > Templates and search for the following key words:

These are the templates that can be edited:

  • Enrolment Form Centre Policies
  • Enrolment Form Declaration
  • Enrolment Form Fee Details
  • Enrolment Form Header
  • Enrolment Form Holiday Break
  • Enrolment Form Parent Declaration
  • CATEGORY (I) MEDICINES
  • CATEGORY (III) MEDICINES


Custom fields can also be added to your online enrolment form. 

1. Navigate to Centre Setup 

2. Select Centre Settings 

3. Child Fields



Here you can create custom fields for your families to fill in and provide information. There are two types of custom fields: 

  • General Fields: appear on the child details tab in the child profile.
  • Permission Fields: appear on both the child details tab and the enrolment form. Are yes/no and limited to 12 per form



Blank copies of the enrolment form can be found by navigating to the child profile > Child tab.
At the bottom of the screen there are options to download, save and/or print a copy of the blank form for your centre. 
Here you will also see the option to download, save and/or print a copy of the completed form for the current child.




On 8 Jan 26, the regulation requiring Early Childhood Edication centres (ECEs) to maintain immunisation records was revoked. This means that ECEs are no longer required to keep those records, but can choose to continue that practice if they wish.

The contents of the Health Section within your enrolment form and what is collected from it can be modified via Centre Setup > Centre Settings > Enrolment Settings.




Unticking Show Health Section will remove the Health Section from your Enrolment Form entirely


This will prevent the collection of any of the information contained within this section. However, you are able to customize your enrolment form to collect choice pieces of information that matches your services needs: 


  • Unticking Illness / Allergies will removethe Illness / Allergies section from the enrolment form. 
  • Unticking Special Diet will remove the Special Diet section from the enrolment form. 
  • Unticking Childhood Diseases will remove the Childhood Diseases section from the enrolment form.
  • Unticking Immunisations will remove the ability to record Immunisation data on the enrolment form. 


Removal of these sections within the Enrolment Settings will not prevent recording of these pieces of information within a Child's Health information on their profile after the fact. 


Below are two enrolment forms, one with the Health Section enabled and one with the Health Section disabled.
